Compassionate Care Hospice takes care of patients primarily in their home. Home is defined by the patient and therefore can mean an individual’s house or apartment, a long-term care facility or assisted living.
It is our goal to control patients’ symptoms while keeping them in their home, where they are most comfortable, surrounded by the people and things that are most familiar and meaningful to them.
First Night At Home
Depending upon each patient’s individual needs, we offer First Night at Home, a program designed to ensure new hospice patients receive a little extra support the first few days on service. Staffed for evening or “off hour” shifts, Hospice Aides provide a “tuck in” visit to patients new to hospice and provide weekend personal care support, as needed, to all patients.
