Care is
delivered by a team of professionals and volunteers. Under the
direction of the patient's physician and our hospice medical
director, the team follows a personalized plan of physical,
emotional and spiritual care. The plan is continually reviewed and
adapted to meet the patient's changing needs.
The team includes:
- A nurse to visit as often as needed to ease pain and relieve
symptoms. The nurse also teaches caregivers to care for the
patient at home.
- A home health aide to provide personal care including bathing,
changing linens and helping with housework.
- A social worker to provide counseling services, and emotional
support to patients and families to improve coping and
adjustment/acceptance of their disease and dying process. Our
social workers also help with financial concerns, assist with
insurance issues and connect the patient and family to available
community resources.
- A qualified volunteer who helps with a wide range of
activities of daily living.
- A pastoral counselor to advise and assist on spiritual issues.
- Bereavement Support and Grief
Counseling:
Grief can affect family and friends for a long time. Our concern
for the patient's family and friends continues during the time of
bereavement. Our staff maintains close contact throughout the year
to assist in any way possible.
